  • List of forms needed for routine MOCB meetings and milestones

  • Appointment of Advisory Committee: within one year of starting in your Thesis lab, but preferably by the spring of year 2 in the program.

  • Advisory committee meeting:  For each committee meeting you need to fill out a committee meeting form, with original signatures from your faculty advisors.  Remember - you need a meeting each year.

  • Learning Outcome Assessment (LOA) Forms
    Note: All 3 LOA forms are in one document:

    • Pre-prelim meeting: please have your committee fill out the Pre-prelim LOA
      At the Pre-prelim meeting it is best to present an outline of what you plan to write in your proposal.  If you have not had a recent committee meeting, you should give a 20 minute talk, but if you have had a recent meeting, the talk is not necessary.  Note: the pre-prelim meeting is not required, but I highly recommend doing it.

    • Prelim exam:  Form on Admission to Candidacy.  If this is your yearly Advisory committee meeting, you need that form as well.  Prelim LOA form.

    • Thesis Defense: Thesis defense form from the Graduate School.  Advisory committee form if applicable.  Thesis Defense LOA
